Once, three tourists - a Swede, an Italian and a British woman - were brought in dead. The latest arrival was a young Mumbai businessman who snorted too much coke and was brought to the clinic frothing at the mouth. Unable to cope with the steady flow of patients, the 62-year-old psychiatrist has added another wing with 70 beds to his clinic. "Some foreigners are so big in size that they create problems," he sighs.

They are the least of the worries for a state firmly in the grip of drugs and organised crime.
